Reminder: Attachment Policy for Dental Claims
As part of our ongoing commitment to providing quality care and ensuring the integrity of our claims review process, Unum has attachment requirements for certain Current Dental Terminology (CDT) codes. To ensure timely and accurate processing of claims, the required attachments should be submitted with all pre-determinations, claims, and appeals. Please be advised that failure to submit the required attachments will result in claims denials or delays. Unum attachment requirements are outlined below:
